Safe Hip Replacements – Be Informed, Be Safe!

Several hundred thousands in the USA go under the knife for hip replacements every year. Research into the material used in implants and the techniques used in surgery have greatly revolutionized hip replacement surgery. With choices available in the type of implant and the replacement technique employed, the surgeon plays an important role in educating about safety and helping the patient make informed choices for safe hip replacements.

A safe hip replacement addresses several aspects about the type of implants used. Safety involves durability of the implant. The newer “metal on metal” and “ceramic on ceramic” implants are more resistant to wear and tear. This is important in younger patients undergoing hip replacements as a more durable implant will save a repeat replacement many years later. Further, implants with smaller heads may be used in mini-incision replacements which might make the patient more prone for a hip dislocation in the future.

Mini-incision replacement techniques on hips are aimed at decreasing the cost of hospitalization and minimizing discomfort to the patient during the post-operative period. This technique involves alignment of the parts of the implant not under the direct vision of the surgeon as in the standard replacement technique but under two-dimensional fluoroscopic image guidance as displayed on a TV monitor. Mal-alignment is a safety issue as it increases the risk of a hip dislocation. Presence of residual bone debris in the area of the implant may cause additional wear on standard “metal on plastic” implants.

Patients undergoing mini-incision replacements on hips are being discharged as early as 24 hours after the replacement. In such situations, patients need to understand clearly safety risks, the plan of post-replacement weight bearing on hips, and activity after discharge.
